From hospital to community: practical implementation models for industry

The NHS needs to shift patient care increasingly out of hospital to make care more affordable, accessible and timely. For pharma and medtech partners, enabling new patient pathways to alleviate the pressures on secondary care and accelerate patient access is of paramount importance. 

This webinar explores how industry and NHS partners can establish remote and community-based services that maintain clinical standards while removing logistical barriers for patients. We will share insights from two CHASE-managed projects that successfully transitioned specialist care from local hospitals into primary care settings. These initiatives provide a practical framework for how industry can support service redesign to meet current system needs. 

This 45-minute session will cover: 

  • Establishing local specialist services: learn how a regional service was set up from scratch to provide local diagnostic workups and treatment initiation, reducing travel burdens for vulnerable patients. 
  • Pharmacy-led monitoring models: examine how moving routine monitoring to clinical pharmacists in primary care can significantly increase protocol adherence and safeguard at-risk patients. 
  • Creating capacity in secondary care: understand how community-based models allow consultants to focus on complex cases by streamlining routine reviews and stable patient management. 
  • Operational implementation: grasp the requirements for recruitment, training, and oversight when launching complex service initiatives in a challenging environment. 
  • Scalable service frameworks: how to use pilot data and real-world evidence to create replication packs for national adoption.
