WEBINAR: From hospital to community: practical implementation models for industry

About this event

NHS demand cannot be met without service redesign. This webinar provides a blueprint for shifting specialist care out of hospitals to create measurable capacity where it is needed most.

We examine how industry and NHS partners can implement remote and community-based services that maintain clinical standards while removing logistical barriers. Using insights from two CHASE-managed initiatives, we demonstrate how transitioning specialist care into primary care settings directly addresses patient backlogs.

The 45 min session will give implementation insights for Industry and NHS partners:

  • Releasing patient backlogs through remote access: learn how a remote nursing service freed up hospital clinic capacity, allowing consultants to focus on new patient starts and complex cases.
  • Pharmacy-led monitoring models: examine how moving routine specialist monitoring to clinical pharmacists in primary care increased blood test protocol adherence from 52% to 94%.
  • Operational safety and governance: understand the frameworks required for recruitment, training, and clinical oversight when moving care out of the traditional hospital environment.
  • Establishing local specialist services: review the setup of local clinics designed to reduce health inequalities by providing diagnostic workups and treatment initiation closer to the patient.
  • National scalability: how to utilise pilot data and real-world evidence to develop replication packs for wider adoption across neighbourhood teams
