About this event
According to the American Heart Association (The Association), cardiovascular disease (CVD) is listed as the underlying cause of death for over 874k patients in the U.S. As healthcare professionals, you know that the best way to combat the rate of cardiovascular disease and subsequent illness is identifying ways to prevent it. Unfortunately, many at-risk patients may not have access to the care they need at the time they need it most – whether that’s because they live in a Health Professional Shortage Area (HPSA) or in a rural setting or, they simply don’t have the resources or support to get to their in-person care visits.
Digital care solutions are quickly turning the tides on how, when, and where patients can access the care that they need. Advanced technology solutions, such as A.I., are giving health plans and providers a new way to integrate scalable, personalized care experiences that empower patients to take control of their health while providing care managers and providers more insight into their patients’ behavior and how its shaping outcomes.
This fireside chat will discuss the intersection between cardiovascular care and technology; what our experts are seeing in the space today, how organizations like The Association’s Center for Health Technology & Innovation (CHTI) are working with technology companies to improve care access, quality, and equity, as well as what the future of tech looks like for health plans, providers, and patients.
